Medical & Liability Release

Authorization of Medical Care: In the event that my child is injured while under the care of the Natomas Awana Club and requires medical attention, I hereby consent to and will be responsible for any reasonable medical treatment as deemed necessary.

I understand that participation in Awana carries certain physical risks and do hereby release Natomas Awana Club, Encounter Church Sacramento, Awana Clubs International, and their representatives from any liability due to accident or injury incurred by my child.  In the event of an accident or other emergency, when a parent/guardian is not available, I hereby authorize a representative of Natomas Awana Club to make such arrangements as s/he considers necessary for my child to receive medical, dental, or hospital care, including necessary transportation. Under such circumstances, I further authorize the physician named above to undertake such care and treatment of my child as s/he considers necessary. In the event said physician is not available, I authorize such care and treatment to be performed by any licensed physician, dentist, or surgeon.

THE UNDERSIGNED PARENT/GUARDIAN FULLY UNDERSTANDS S/HE IS RESPONSIBLE TO PAY ALL COSTS INCURRED AS A RESULT OF THE FOREGOING.

I also authorize Natomas Awana Club and Awana to publish photos on their websites and brochures for promotional purposes.
